Introduction How many of these questions about statin drugs, the top-selling drugs for lowering elevated levels of cholesterol and C-reactive protein, can you answer? •         Do I need to lower my cholesterol level? •         Do I need to take a statin drug? •         Which statin is best for me? •         What is the right dose for me? •         What are the side effects and how can I avoid them? •         Are there long-term risks with statins? •         If side effects occur, what can I do? •         Are there alternative methods I can use instead of statins? •         Are there other things I can do to preserve cardiovascular health? •         What is the best heart-healthy diet for me: low-carb or low-fat? •         How important is exercise, and how much is necessary? •         How can I save 50 percent or more off the high cost of statin drugs? These are important questions for the 100 million Americans with elevated cholesterol. These questions are even more important for the 20 million Americans who take statin medications―Lipitor, Zocor, Pravachol, Mevacor, Lescol, or Crestor―and for the 25 million more who are slated to be taking statins soon. Taking statins is simple, right? Elevated cholesterol = statins. Yet statins are potent drugs that can cause serious side effects. Are doctors prescribing statins with the care needed to use them effectively and safely? Are doctors carefully selecting the right statin at the right dose for people as individuals? Are doctors thinking about how to reduce people’s risks of side effects and long-term toxicities? Are doctors offering you and others ways for keeping statin costs as low as possible? Are they informing you about other safer, proven-effective, less expensive alternatives? For many statin users today, the answer is no, no, no, no, and no. Statins help millions of people, but like all drugs, they can also cause harm. Medicine’s most respected pharmacology reference, Goodman and Gilman’s Pharmacological Basis of Therapeutics, states: “Any drug, no matter how trivial its therapeutic actions, has the potential to do harm.” Statins’ effects are not trivial. Statins are very powerful chemicals that exert effects in every cell of your body. Many of these effects are good, but some can be bad. How can you maximize the benefits while minimizing the risks? Just as when buying a car or choosing a college for your child, the more information you have, the better your chances are of doing it right. What information do you need to take statins safely and effectively? That is what this book ­provides. Most doctors will simply tell you that statins are safe and hand you a prescription. Yet statins have many common side effects that cause physical discomfort or mental impairment in millions of people. Sixty to seventy-five percent of people started on statins stop taking them. The average patient lasts only eight months on statins. Why? Side effects. Inadequate effects. Cost. These are the issues that come up again and again when I give seminars, speak with people, or read their letters. These are the problems that mainstream medicine has not learned to overcome. But they can be overcome―indeed, they can be prevented―which is why I have written What You Must Know About Statin Drugs & Their Natural Alternatives. I am pro-medication and pro-statins, but my first responsibility, like all doctors, is to “Do No Harm.” Sometimes doctors cannot avoid doing harm, but this is not the case with statin drugs. Statin risks can be dramatically reduced with a treatment model that emphasizes precision and safety, as this book will describe. Statins can be very effective drugs―if given for the right reasons to the right persons at the right doses with the right treatment goals. Unfortunately, millions of people are prescribed statin doses that are too strong for their treatment goals, provoking side effects. Fifteen to forty-two percent of people given statins get side effects. That’s 7 to 15 million people. These side effects may be considered minor by doctors, but abdominal discomfort, muscle or joint pain, or memory impairment are not minor to you. And some statin side effects are not minor even to doctors: severe muscle pain or weakness, nerve injuries (some permanent), acute muscle breakdown, kidney and liver irritation or toxicity, and rarely, death. Moreover, when 60 percent to 75 percent of people needing statins quit treatment, no one benefits. The long-term consequences of premature heart attacks and strokes harm not only patients and their families, but remain a costly, unsolved problem for hospitals, healthcare systems, insurers, and even the pharmaceutical industry and U.S. Food and Drug Administration. The good news is that most of these problems are preventable, but only if you and your doctor know how. This is explained in Chapter 1, Avoiding Statin Overtreatment or Undertreatment: The Precision-Prescribing, Safety-First Method. The precision-prescribing, safety-first method will allow you to maximize the benefits of statins while minimizing the risks. The most important question, of course, is whether you need statin therapy at all and, if so, how much? Chapter 2, How Much Cholesterol Reduction Do You Really Need?, answers these questions. Chapter 2 briefly explains the role of cholesterol, low density and high density-cholesterol (LDL-C, HDL-C), and C-reactive protein (CRP) on artery-clogging atherosclerosis and cardiac disease. Because of confusion about treatment goals, many doctors prescribe statin doses that are overly aggressive for people’s requirements. This excessive treatment brings little extra benefit in exchange for significantly increased risks. It’s not a good deal. That is why it is important for you and your doctor to know how much cholesterol-lowering is really beneficial, so treatment can be tailored to your needs and goals. Chapter 2 also tells you about five other risk factors that are often overlooked. These risk factors explain why half of the people who get heart attacks have normal cholesterol levels, so you want to know about them. Chapter 3, The Right Statin at the Right Dose, explains why the standard initial doses of statins are unnecessarily strong for millions of people. Because of the omission of important information from package inserts and the Physicians’ Desk Reference (PDR), many doctors, not intentionally, overshoot the mark when prescribing statins, giving patients very potent doses when mild ones would do. Chapter 3 describes who is most likely to benefit from the precision-prescribing method and what your goals should be when first starting treatment. Chapter 4, Low Dose Statins―The Evidence, provides several strategies for choosing the right statin at the right dose for you. Chapter 4 presents the scientific evidence on the lower, safer, proven effective doses of each statin. It’s not enough for me to say that lower statin doses work. Medical treatment should be based on scientific proof whenever possible, and that is what this chapter provides. Chapter 5, Why Do People Respond so Differently to Statin Drugs?, explains why different people respond so differently to statin drugs. It explains why some people need strong statin doses while others need just a little, and how to identify each without causing harm. Chapter 6, Women and Seniors― Special Considerations with Statin Drugs, focuses on two groups that require extra attention and information in order to use statins effectively and safely. Few patients or their doctors know about the issues discussed in these chapters, but they are essential for optimal care with minimal risk. Chapter 7, Effective Strategies for Dealing with Side Effects and Other Problems with Statins, answers the many questions that people sustaining statin side effects have asked me. Unfortunately, the medical system isn’t very good about identifying or handling side effects, so I have divided Chapter 7 into seven sections that identify specific problems and offer practical solutions. Many people do not like taking prescription drugs. If you’re one of them, Chapter 8, Effective Alternative Therapies for Reducing Cholesterol and Other Risk Factors, is for you. There are several proven-effective alternative therapies worthy of consideration that cost much less than brand-name statins. Because making the right decisions about reducing cholesterol and other risk factors will have a major impact on your long-term health, I always recommend that you and your doctor make decisions about these alternative methods together. Whether you take a statin or an alternative therapy or do nothing at all, there are four nutrients that everyone should consider for preventing cardiovascular disease. Chapter 9, Essential Nutrients for Cardiovascular Health, describes them and their importance: omega-3 fatty acids (fish oils), coenzyme Q10, folic acid, and magnesium. Most people are sorely deficient in these nutrients, whose vital roles in maintaining a healthy heart and vascular system are backed by extensive scientific study. Nutrition, of course, is also a big part of the equation for heart health. Yet there is so much confusion today. Low-carb or low-fat? Atkins or Ornish? Chapter 10, Which Heart-Healthy Diet Is Right for You?, provides original, clear guidelines to help you determine the best diet for you. This chapter also discusses exercise: the part it plays and the surprisingly little amount that’s required to maintain heart health. Obviously, you cannot take statins if you cannot afford them. Their high cost is another reason that people avoid taking statins, quit taking them, or take them less often than necessary. Chapter 11, How to Cut Statin Costs by Fifty Percent or More, explains why statin costs are so high and provides several strategies for substantially reducing the cost of treatment. This chapter also gives you a comparative analysis of statins versus natural alternatives, a cost analysis most doctors or healthcare systems have never seen or considered. If cost is an issue for you, you need to know about all of these options. The book concludes with Putting It All Together, an overall strategy for setting your priorities and reaching your goals. Heart disease and strokes can be prevented. Vascular health can be preserved. By defining which steps you need―statin drugs or natural alternatives, essential nutrients, diet, and exercise―and how to easily employ them together, you and your doctor can decide how to improve the quality and extend the duration of your life. I have written What You Must Know About Statin Drugs & Their Natural Alternatives to provide original, practical information that you will not find in statin package inserts, the PDR, or medical textbooks or consumer references on statins or medications. Most books on cholesterol discuss statins with the usual pharmaceutical industry slant, which is the information that results in millions of side effects and millions of people quitting needed treatment. Such methods are wasteful and harmful. This book is much different: It is a primer on how you can decide whether you need treatment and, if so, exactly how much you need and how to accomplish it safely. The best treatment―statin or alternative―is the least powerful treatment that works. This book helps you define exactly what that is for you. In addition, unlike other books, this book provides information about essential nutrients that are vital, yet usually overlooked, for preserving cardiovascular health. Most doctors think they know everything they need to use statins properly, but they don’t. I learned similar lessons the hard way when I treated patients. There is a tremendous amount of important information that doctors do not get through their usual sources, most of which comes from the pharmaceutical industry, and that you do not get through the media and typical bookstore references. Most doctors and consumers do not know, for example, that medication side effects are the number four leading cause of death in America year after year, decade after decade. They also do not know that medication reactions cause more than 106,000 deaths, 1 million hospitalizations, and 2.2 million severe, often permanent disabilities each year. Worse, these numbers may be underestimates because they represent only hospital patients, not the 100 million outpatients taking prescription drugs. Most doctors and consumers do not know that many of these problems are preventable. These problems can be prevented, but only if you and your doctor have the information needed for matching your treatment to your actual needs and tolerances. This is the basic principle of the precision-prescribing, safety-first method. You have a right to information that makes it possible to obtain optimal care with minimum risk. This right is known as informed consent. This right applies to medication treatment just as it does to surgery. But to fully exercise your right of informed consent, you must receive enough information to make an intelligent choice. Most people do not receive enough information about statins to fully exercise their right of informed consent. Indeed, most people do not know that they have choices―many choices―about different doses and alternatives when considering statin drugs. Many of their doctors don’t know this either. Unless you receive information about how to maximize the benefits while minimizing the risks and costs of statins, you have not received informed consent. You aren’t alone. Studies show that only a small percentage of patients receive sufficient information to make an informed consent in doctors’ offices. Statins can do a lot of good or cause serious harm. The difference depends on the decisions that are made based on the information available. The information available to most patients and their doctors today is inadequate to make informed choices for using statins safely and effectively. This book is a no-nonsense presentation of the facts about cholesterol. Finally I have seen something that actually tells the truth instead of twisting everything into a self-serving set of misleading statements designed to sell a product.This book is a refreshing change. Dr. Cohen gives both sides of the research and carefully explains what the different studies actually mean.He makes no broad sweeping generalizations about statin drugs. Some people can benefit; others can use alternatives.He explains all the options and why.Very important. Very informative.

This book is for every person with a little bit of elevated cholesterin who's doctor tries to force Statins on them (like mine does). It's a book that can save you money and your mental health!

I have read several books on the subject of treating high cholesterol. This book is by far The most comprehensive. It goes into great detail about statins, dosages, side effects, natural alternatives, etc.If you only purchase one book on the subject, this is your Bible!

I liked this book because the author effectively bridges the gap between mainstream and alternative medicine for cholesterol management. He is more accepting of statins than I like, but he rates them as a last choice after lifestyle and supplements. That's good. He also advocates a "start low, go slow" prescription plan which makes sense. Some information is totally obsolete, especially the parts about the costs of statins. Red yeast rice that is available in the USA no longer contains any lovastatin in most cases.

This book is a must-read for those with high cholesterol. You really do have to be your own health advocate and not go only by what your doctor recommends. I agree with Dr. Cohen in that a lot of doctors are "in bed" with pharmaceutical companies and are given big perks to push their drugs at levels which aren't necessarily safe or effective.I have moderately-high hereditary cholesterol levels and have tried many statin drugs in the past - all with bad side-effects which exacerbated my fibromyalgia (muscle pain, weakness). After reading this book, I've quartered the statin pills my doctor gave me and have had no bad side effects. Saves money, too! Dr. Cohen's motto of "Start low and go slow" is good advice when it comes to statins.

This is a great book written by a doctor who gives an in-depth look at the various types and brands of statins and how to dial in a Cholesterol lowering dosage for each individual. He also explores alternatives, nutrition and diet as a natural way to lower Cholesterol, LDL and HDL. Anyone taking Statins or considering starting would find this a good read.

This MD breaks the mold of obedient robots pushing doses regardless of efficacy. So many drugs are great, but the dose need not be so high. I gifted my cardiologist with a copy, begging him not to be insulted lol

This book is extremely well written and provides a good basic introduction to ABAP and the fundamental development tools in R/3. I was disappointed however to reach day 21 without creating any interactive screens and having no mention of creating batch inputs. If this were expanded to 42 days it would definetely get 5 stars. With this book, SAP Help, and the Gareth M. De Bruyn book Advanced ABAP Programming for SAP most of what I needed was available but sometimes a struggle to find.

I think that the practise codes in CD should be made unicode compliant. This will greatly help the beginners.

This is best book I came across in terms of concepts.For every concept there is a practical example .This is best way to learn instead of reading long boring sentences.This book is meant for beginners and those who already used languages like cobol,java but learning ABAP.However this covers only basics of ABAP in reporting.It doesn't cover dialog programming which is big area.However it is best beginning for someone starting ABAP/SAP career and I highly recommend it.Please note the version is abap 3.0 which is too old (now we have 4.7).However all the examples work in 4.7 so that is not big issue.

Overall Rating: ***** (Excellent)I have personally taught more than 32 weeks of the SAP ABAP/4 Partner Academy courses and read several books on ABAP/4.This book is definitely "best in its class" covering material from SAP's BC400 and BC430 courses (week 1 of the ABAP/4 Partner Academy) and beyond. Ken is a master at making complex concepts easily understood. The book is full of helpful diagrams, R/3 screen captures, program cutouts (explained line by line by Ken), and helpful tips.The end of each chapter concludes with a "Summary", a table of "Do's and Don'ts", "Questions and Answers", and a "Workshop" that includes a "Quiz" and, where appropriate, one or more "Exercises". This is truly an excellent training tool, going far beyond the typical "reference" books I have read in the past.The CD-ROM at the back of the book includes an "uploadable" copy of the source code of programs used in the book, 58 Lotus ScreenCams that demonstrate many key tasks on a live R/3 System (extremely helpful for those who do not have access to an R/3 System), four very useful utilities for ABAP/4 developers (written by the author), and even a trial version of a "German-to-English" translator (seasoned ABAP/4 programmers will understand the value of this tool for interpreting German-only documentation found in many SAP function modules!).Warning to all ABAP/4 instructors: get your hands on this book quick before your future students do - they may end up knowing more than you do!Overall - an excellent resource!

I found the book EXTREMELY heavy on data dictionaries and internal tables, but VERY light (read - non existing) on external file imports or exports. It didn't have a section on BDC's. Functions & parameters were at the very end of the book (2/21 days!). One of the things I found most frustrating was that the book never indicated the version of SAP it was running on. While not a deal breaker, screen cams were hard to follow because they didn't look anything like my 4.5B system at work! If you want to learn about data I/O & reporting, I would suggest spending your money on "Introduction to ABAP/4 - Programming for SAP" instead.

Very good for beginners, sometimes better than online help for advancedI have started with this book my ABAP adventure. This book gives me overview of all-important ABAP command and showed me some good ABAP programming technique. Later when I can't find something in on-line help I am coming back to this book and I can always find what I need. I like very much a part, which explain how internal tables are working.

I'm not a full-time ABAP programmer, SAP BW is my day job. But often, I have to pick up a particular technique in a hurry. That's why the worked-through examples are so helpful to me, and the indepth treatment of internal tables. Also valuable were the conversion & string examples. All in all, a very nice approach for my needs.

I bought the book since I am a beginner in ABAP. Completely disappointed.* The book is OLD and fails to mention the SAP version it deals with.*It does not explain ABAP actions but rather shows and tells. How can one learn from this. For example a "logon client" is described as "... the number that the user types in the client field." No mention of the significance of clients for security and different business needs.*On top of that, the CD with the book claims to be the best instrument in teaching and showing ABAP but it does work on any system later than 95 or NT.All in all a very poor book.

This is not my usual book to read. We chose it in book club, but I loved it. Great writing and really made me think about how we treat each other as human beings. Also gave me more empathy for the troubled and bullied kids out there. Cried for the last 3 or 4 chapters. It is truly a heart wrenching story (situation) for all those involved. I certainly re-evaluated some of my opinions of people and reminded me not to judge others when I don't know all the facts. This was a book I could not put down!

I've read this novel twice. Once a few years ago and again recently for a book club. This novel is timely and very well written.I am in my late 60's and glad that my childrearing days are well behind me. Parenting is very challenging, especially the teenage years. Although people always look to the parents when a child exhibits antisocial & troublesome behaviors but while the parents may be the primary caregivers the child comes in contact with many people who help shape their personalities. This book examines every aspect of Peter's life experiences and leaves us to decide which influences contributed to his attitudes.

As an avid reader and fan of Ms.Picoult's stories, I recommend this story of a family that could be your neighbor or family. A regular couple living in ANYTOWN, USA, Dad,a firefighter, a stay-at-home Mom and 3 kids....a boy, and two girls. However, the 2nd child, a girl is very ill and needs constant medical care, hospitalization and surgery. Their lives revolve around this child and story takes place over a few years. No Spoiler here. Ms. Picoult draws us in to their lives and all the drama it entails.

I'm pretty shocked that parents had a problem with this book being assigned to high school students. It's completely relevant to today's youth. What's surprising about the attempt to ban this book was that parents had a problem with the sex scenes, not the violence. Sure, a kid shoots other kids and that's no problem but even the mere mention of sexually active high school students sends parents into a fit? Grow up. There are much larger issues in this book, as in life, and sex doesn't always have to be the first thing on everyone's mind. Come on adults--let's set a better example of our kids. Read this book to remember what high school was like and see it through your kid's eyes. Talk to them about bullying, and about sex, instead of trying to ban a book that can easily open the conversation.

Three stars for Ms. Picoult's courage to write on such a difficult subject, and for her mostly appropriate portrayal of the various age-groups and the genders. Minus two for what came across as sometimes a juvenile writing style (her book Plain Truth was much better imho), and for the cutoff at the end that left too much gap between what occurred throughout the story and the abrupt ending she left hanging for some of the characters. She tried to tie it all up in the final chapter, but it did not answer many questions, nor did it do justice to the buildup during the story.

Wow! What insight into the minds of those who are so relentlessly victimized that they finally snap and become victimizers. I've worked with adult clients who say, based on their own experiences of being constantly bullied throughout school, that the victims of mass school shootings deserve it. I don't agree, of course, but I understand the rage. Having worked in the public school system, I can guarantee you that the preferred method of dealing with bullying is to ignore it. We cannot let this continue. Bullying destroys the self-esteem of people for many, many years, if not a lifetime. I was bullied throughout elementary school and, thank God, my parents moved so I could get a fresh start where I was accepted and even enjoyed a certain level of popularity ... so it isn't always the "victim's fault" that they are picked on. If your child is bullied and your school won't deal with it, (1) turn them into their superintendent, (2) turn them into your state's education agency, (3) insist that the district allow your child to change schools, (4) if you must, move, (5) arrange for QUALITY home school, (6) pay for private school, (7) get your child counseling with a counselor who enjoys playing with younger kids or talking to older kids and teens, but DO SOMETHING because school bullying causes lifelong self-esteem issues that affect academic success, careers, and whether or not your child becomes a bully or marries one.

Being a retired teacher, I was drawn to this story. (I also really have enjoyed reading many Jodi Picoult stories). Too many children are bullied in today's world. Too many children try to hide their soft, quiet personalities just to try and blend in with those who bully others. Jodi describes the world those "picked on kids" live in and she does it with her emotionally charged writing style. Well done!

This has to be one of the thickest books on my shelf but I read it in a couple of days. Couldn’t put it down. Jodi takes a shot at a real life headline story and manages to pull her audience into this powerful thought provoking scenario which makes you second guess yourself and question your judgement. She brings front and center, an issue (school shooting) relevant in our society today. She makes you empathize with the villain. And she teaches you a powerful lesson, that there’s more to a story beyond its headlines. I couldn’t put the book down until i read the last line. She conjures human emotion like no other author. She motivates you to step outside your preconceived notions and judge things, people, situations, for what they really are. You should read this book as a parent, as a teen, as an adult or as an educator.
